Death like everything else is a process of change. The Bardo Thodol ( commonly known in the west as the Tibetan Book Of The Dead) is said to describe the process in great detail, from the breath stopping to the taking of a new rebirth. And the intermediate states. * +

Various high lama's have described the entire process and often prior to death they leave letters to their trusted followers that state where, when, and to whom they will take rebirth..even down to the address of the new parents they will be born to. Sometimes they state they will take rebirth as several beings at once. This concept only makes sense if a person understands the concept of rebirth and not reincarnation. * +

Allah's Messenger said, Anyone who dies, Worshiping others along with Allah will definitely enter the Fire. (sahih al-Bukhari 1238) or (sahih A-Bukari Vol. 2, Book 23, Hadith 330)
