Apparently the captain wasn't at fault and did everything that he was supposed to do. It's believed there was a local harbour/river pilot on board, whose job it is to navigate the larger ships out of harbour and guide them through the river's ever shifting deepwater shipping channels without running aground on a sandbank, that would/should have been at the helm at the time according to the standard procedure. * +
