I believe I read that they can be held in place with velcro. They do specially designed undrwear so they don't stand out when hidden under clothing and there are all sorts of straps and other things to secure them in place. I don't think you even need to wear the bag all the time nowadays, they are constantly improving the procedure and the equipment to make them easier to live with, some of the newer ones you can take off whilst you sleep and really only need to attach the bag when you need to 'evacuate', the rest of the time the stoma is sealed off with some sort of cap. * +
